Permission Slips

For privacy reasons all permission slips for Troop 119 activities will be posted in the private site.  

All permission slips and fee's will be due 2 weeks prior to the date of activity listed ( unless otherwise noted).

 

 

Medical Forms

Medical forms may be required for participants of certain activities.  See the event description or contact your leader for details on which form(s) may be required.


Spring Camporee

All scouts and leaders attending MUST have a new medical form on file.  Complete parts A and C (No doctor sign-off required).

Resident Camp

Every Scout, leader and parent attending MUST have on of the new medical forms on file.  Parts A, B and C (Doctor sign-off required on Part B).

 

 

Summer Camp Commitment Letters must be completed and turned in with deposit NO LATER THAN January 13, 2011.
